Go to our donor recruitment website at www.123donate.com. Click on the laboratory location nearest you and fill out the short application online. Once reviewed by our staff, you may be asked to fill out a longer, more detailed online application.
After reviewing your online application forms we will determine if you qualify to procede to the next part of the process. If you do, you will be contacted for an interview. The interview is the first part of the on site screening process. The entire screening process is typically completed in about four weeks. Interviews are conducted Monday - Fridays between 8:00 AM - 12:00 Noon. At the time of the interview, your application will be reviewed with you. The anonymous donor program will be explained to you in its entirety and any questions you may have will be answered. You will then be asked to produce a specimen.
The next part of the screening process involves evaluating your sperm quality. You will be asked to produce three to four sperm specimens for evaluation of both fresh sperm quality and cryo-survivability.
If the applicant's semen quality passes our strict standards, then medical and infectious disease testing will be performed.. The donor applicant is required to give blood, urine, and sperm specimens for the genetic and infectious disease testing. In addition, a general physical examination will be completed. It takes approximately three weeks before the results are received on the submitted specimens. Once the final results are reported and all results are negative, the applicant will be accepted into the program and be asked to start donating on a regular basis as an active donor.
The first specimen produced as an active donor will be considered his official start date and his six month commitment will begin from this date. At the end of the six month commitment,he will be reassessed as a donor. If for some reason he choses to stop donating or is asked to stop donating, this would be considered his official stop date. The donor will be required to provide blood samples at three months and six months from his official stop date in order to release quarantined doses. Each dose that is produced and inventoried for distribution at Fairfax Cryobank is quarantined for six months.
Fairfax Cryobank is dedicated to cryopreserving anonymous donor sperm for infertile patients. Although most donors choose to remain anonymous, we have recently started a program called ID Options where registered offspring will be offered the opportunity to contact their sperm donor when they reach 18 years of age. Donors accepted into the program will be asked if they would like to be part of this new ID Options program. It is strictly a voluntary choice. A donor may select to remain in the anonymous donor program only. We are very committed to our donors and require the same commitment from them. We look forward to hearing from any interested prospective donors.
