- Terms of Use Letter - Effective, November 1, 2006. For new patients, Fairfax Cryobank must receive a signed copy of this Terms of Use letter via fax, mail or scanned image into email at least one business day prior to shipment of any Specimens. This document explains what you can expect from our program and what we can expect from you when purchasing our semen and other services.
- Must Read Before You Buy - gives advice on commonly asked about topics including donor availability, our specimen quality standard, our limits on donor distribution and the importance of checking the donor medical status over time once vials are shipped off site.
- Identity (ID) Option Birth Registration Form- must be completed after the birth of a child from a ID Options donor if access to donor information at age 18 is desired
- Identity (ID) Option Patient Agreement- must review instructions and then download the form, sign and return
