{"id":27362,"date":"2015-05-18T13:52:18","date_gmt":"2015-05-18T13:52:18","guid":{"rendered":"https:\/\/fairfaxcryobank.com\/transfer-of-ownership"},"modified":"2015-05-18T13:52:18","modified_gmt":"2015-05-18T13:52:18","slug":"transfer-of-ownership","status":"publish","type":"page","link":"https:\/\/fairfaxcryobank.com\/ie\/transfer-of-ownership","title":{"rendered":"Transfer of Ownership"},"content":{"rendered":"\t<ul class=\"faqs\" >\n\t\t\t<li class=\"faq\" id=\"faq-block_5d1123ca8ae79\">\n\t\t\t<div class=\"-question\">What is a Transfer of Ownership?<\/div>\n\t\t\t<div class=\"-answer\"><p>Patients who have Fairfax Cryobank semen samples stored onsite with Fairfax Cryobank are able to transfer their samples into another person\u2019s name for use by participating in what we call a Transfer of Ownership.\u00a0 This allows Fairfax Cryobank to properly track the owners of our semen samples to ensure that we are able to closely monitor the number of Family Units for each donor.\u00a0 This also allows us to create a letter for a second parent adoption for our patients as are commonly requested.\u00a0 In addition, new owners of vials will receive medical updates as their contact information is now in our system.<\/p>\n<p>Please be aware that in order to proceed with a transfer of ownership for a donor that is Sibling Only, the recipient of the vials must have a reported birth on file.\u00a0 Likewise, if you are transferring vials from an ID donor, the recipient will need to complete the ID Donor form if they have not already done so.<\/p>\n<\/div>\n\t\t<\/li>\n\t\t\t<li class=\"faq\" id=\"faq-block_5d1123ca8ae79\">\n\t\t\t<div class=\"-question\">How does a Transfer of Ownership work for vials stored at Fairfax Cryobank?<\/div>\n\t\t\t<div class=\"-answer\"><ul>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Current owner completes the Transfer of Ownership form<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Transfer of Ownership form must be notarized<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">New owner completes the Acceptance of Ownership form<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Payment of a $300 transfer fee as well as applicable shipping fees<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">New owner completes the Billing Agreement and Donor Semen Storage Client Agreement<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">All documents and payment returned to the Cryobank in one package<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Upon receipt of the documents, we will begin the Transfer of Ownership and send update storage letters to confirm completion of your transfer.<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">The new owner will then set up a storage account for the new vials.<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">The vials will then be ready to ship on request.<\/p>\n<\/li>\n<\/ul>\n<\/div>\n\t\t<\/li>\n\t\t\t<li class=\"faq\" id=\"faq-block_5d1123ca8ae79\">\n\t\t\t<div class=\"-question\">How does a Transfer of Ownership work when my vials are stored at my clinic (referred to as a Soft Transfer of Ownership) ?<\/div>\n\t\t\t<div class=\"-answer\"><ul style=\"margin-bottom: 0;\">\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Current owner completes the Transfer of Ownership form<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Transfer of Ownership form must be notarized and a signed Terms of Use must be signed<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">New owner completes the Acceptance of Ownership form<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Payment of a $100 transfer fee<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">All documents and payment returned to the Cryobank in one package<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0; margin-bottom: 0;\">Upon receipt of the documents, we will begin the Transfer of Ownership and send confirmation of the completion of your transfer.<\/p>\n<\/li>\n<li>\n<p style=\"margin-top: 0;
