Updates - Family Medical HistoryUPDATE Nov 2025: Fairfax was notified that a donor conceived child was diagnosed with a ZBTB18 related neural development disorder. Records have been requested to investigate this report and to assess next steps. Additional information will be provided as it becomes available. UPDATE Dec 2025: Genetic testing was ordered on the donor to determine if he carries the specific variant in the ZBTB18 gene that was identified in the affected child from the Nov 2025 update. The donor does NOT carry this variant. As such, the variant in the child is believed to be new to that child and not inherited from the sperm or the egg source. We do not believe there is an increased risk for other donor-conceived children to have this variant in the ZBTB18 gene. This donor has been moved from Investigation to Inactive. UPDATE April 2026: Fairfax was notified that a child was identified to have mildly enlarged ventricles in his brain at 32 weeks via ultrasound. Previous prenatal assessments (NIPT and fetal ECHO at 20 weeks) were reportedly WNL. No amniocentesis was conducted. An MRI during pregnancy and after confirmed stable mildly enlarged ventricles. A possible narrowing of an unspecified vessel in the heart was suspected at 36 weeks gestation but a post birth ECHO was WNL. The baby was deliveried via planned C/S. Right after birth, the baby had trouble breathing and eating, resulting in poor growth, micro retrognathia, a sacral dimple and low-lying conus, low set ears, undescended testicles with chordee, and an optic disc with unconfirmed hypoplasia. No underlying spinal issue was detected. The baby is currently in the NICU and followed by ophthalmology, GI, neurology, urology, neurosurgery, genetics, cardiology, and endocrinology. Genetic testing revealed a variant of uncertain significance in the child - (c.674G>A , p.(r225q)) - in the ARHGEF40 gene. Per an email, the egg source was not identified to be a carrier of this variant. Results were obtained. Donor has been placed in Investigation while this case is reviewed by the Genetics Team. Per the client, she has a daughter from this sperm donor with no reported health concerns. UPDATE May 2026: Genetic testing was ordered on the donor to assess if he carries the variant of uncertain significance (c.674G>A , p.(r225q)) in the ARHGEF40 gene that was previously identified in the child. UPDATE May 2026: The donor's genetic test results showed that the VUS (c.674G>A , p.(r225q)) in the ARHGEF40 gene mentioned above was not identified in the donor. As such, this VUS appears to be a new variant in this child and not inherited from this donor. The chance for other children conceived with the help of this donor to have this VUS is not believed to be increased.
