| Updates - Family Medical History | UPDATE AUGUST 2025: Client reports pregnancy conceived with the help of donor 6909. Underwent amniocentesis with whole genome sequencing for standard of care. No ultrasound findings were detected to initiate testing. The results have come back with a heterozygous likely pathogenic variant in TRIM28, which was deemed to be reportable because similar variants have been associated with an increased risk for Wilms Tumor in infancy/early childhood. The risk of malignancy in children with variants in this gene appears to be lower (although not zero) when it is paternally inherited. The variant does not appear to be inherited from the egg source but we are trying to establish if this variant was paternally inherited or de novo. We are currently attempting to contact the donor to arrange testing. UPDATE October 2025: Donor 6906 was subsequently determined to carry the same variant in TRIM28 that was identified in the pregnancy mentioned above. The risk for those conceived with the help of donor 6906 to inherit his TRIM28 variant is 50%. Having a single genetic variant in the TRIM28 gene has been reported in association with sporadic and familial cases of Wilms tumor. Wilms tumor is the most common common pediatric renal cancer and the fourth most common pediatric cancer overall. Ninety percent of Wilms tumors are diagnosed before six years of age with the median age of diagnosis being 3.5 years. Wilms tumor usually presents as an asymptomatic abdominal mass, though some children can experience abdominal pain, urinary tract infections, changes in blood pressure, fever, and blood in the urine. With marked improvements in chemotherapy, most affected children now survive. Inherited variants in TRIM28 demonstrate a parent-of-origin effect where individuals who carry a maternally inherited variants are often symptomatic. An individual was reported with bilateral Wilms tumor and a paternally inherited TRIM28 variant, however additional research is needed to explore a possible link between Wilms tumor predisposition and paternally inherited TRIM28 variants. Update received October 2025: Donor reports the birth of a healthy son; Maternal Aunt #1 has died at age 67, cause of death unreported. |
