Established in 1986, Fairfax Cryobank has over three decades of providing sperm banking services.

For those facing cancer treatment, we have established a reduced rate program to make fertility preservation services more accessible.

Fertility & Cancer-related Therapies

Before you or your loved one begins cancer treatment, ask your doctor about the risks of sterility.

Treatments like surgery, chemotherapy, and radiation may impact your fertility. Opting for sperm storage prior to or in the early stages of treatment offer peace of mind that your journey to biological parenthood is preserved.

When stored properly, cryopreserved specimens remain viable for an indefinite period. Sperm stored for 20 years prior to thawing have resulted in pregnancies, so even if you don’t want children soon, sperm storage is still an option.

How Does the Process Work:

Contact Fairfax Cryobank to set up your initial appointment.

All services can be completed at the initial appointment. You’ll complete the consents and storage agreement to set up your account. A blood draw for required infectious disease testing will be completed. Infectious disease testing can also be completed by your healthcare provider.

We can provide the tests required for storage. You’ll also collect a semen specimen to be processed and frozen. Fairfax will provide a report for specimen frozen. You are welcome to call us with any questions.

Additional semen specimens can be provided at subsequent appointments, if you wish to do so.

In-Patients Sperm Banking Services Available:

We are also able to support those that are in-patient with a semen specimen drop-off option. This can be coordinated directly with one of our Fairfax Cryobank sites.

Agreements and consents can be provided electronically for completion. A family member or friend can drop off a semen specimen for processing and freezing at designated appointment timeframe.

Required infectious disease tests can be done with your healthcare provider. The results of the tests can be sent to Fairfax electronically upon completion with a healthcare provider.

Note:

Process outlined above is for banked sperm to be used in the future with a sexually-intimate partner.

If banked sperm may be used in the future with someone who is not a sexually-intimate partner, including surrogate usage, additional screening is required as a “directed donor”.

This screening can be completed by Fairfax Cryobank, however, does require additional visits and consents. Please specify when making your appointment, if you may be using your banked sperm may be used in the future by someone who is not a sexually intimate partner, so the required screening upfront is completed.

Fairfax Cryobank: Sperm Banking Frequently Asked Questions:

Do your offer sperm banking by mail?

Fairfax Cryobank is proud to offer the Fairfax Sperm Freezing Kit, an overnight collection kit in association with Cryogenic Laboratories, Inc of Roseville, MN.

The Fairfax Sperm Freezing Kit offers men the opportunity to cryopreserve their semen specimens prior to medical treatment, even if they are unable to travel to a sperm bank.

How many semen specimens should I store?

Fairfax Cryobank recommends storing at least two semen specimens 48-72 hours apart before chemotherapy treatment or surgery.

Patients who only have time for one specimen are encouraged to bank their sperm because each specimen is divided into vials and may be used for multiple fertility procedures.

What blood tests are required for sperm banking?

All patients are required to have HIV 1/2 Antibody, Hep B surface Antigen and Hep C Antibody tests completed within the last 30 days or have their blood drawn for the tests at the time of the appointment. For specimens used in the state of California, HTLV I/II and syphillis testing is required in addition to the HIV 1/2, Hep B and Hep C tests. Patients who have had the tests completed within the last 30 days must bring a copy of the test results at the time of the appointment.

How many days of sexual abstinence should I have prior to producing a specimen?

Patients are asked to have abstained from all sexual activity for at least 2 to 5 days prior to the appointment and to have had at least one ejaculate within the last 5 days. This should allow for the best possible specimen and permit a suitable number of doses to be banked for future use.

What forms do I need to complete before my banking appointment?

Please request the required forms be mailed to you before your visit by contacting us. They will also be available at the Cryobank at the time of your appointment if you have not had a chance to receive and complete the necessary information.

Do I need blood testing to store my sperm?

The FDA requires infectious disease blood testing to be performed to release a sperm sample from quarantine.

Required tests are: - Hepatitis B Surface Antigen - Hepatitis C Antibody - HIV 1 Antibody - HIV 2 Antibody

You may store your samples before the blood testing is complete, but will be subject to additional storage quarantine fees. Furthermore, samples without proper blood testing will be required to complete a physician waiver in order to use the sample in the future.

Blood testing may be performed on-site at any of Fairfax Cryobank's locations, or you may provide results from testing at your off-site clinic. Bloodwork from an off-site clinic must have been performed within 15 days of the sample being collected.

How do I interpret my specimen results?

One business day after collection, your results will be sent to the email you provided. The results will include the values of your specimen and reference values from the World Health Organization to compare your results to an average specimen.

If you have questions about your results, you may contact the Fairfax Cryobank lab staff. For questions regarding success rates of procedures and the recommended use of your specimen, we suggest contacting a fertility specialist in your area.

What are the steps for using a Fairfax Sperm Freezing Kit?

The following is an outline of the Fairfax Cryobank Mail-in Sperm Freezing Kit from ordering through future sample use. Specific client situations may require additional steps, but the following items will apply to all mail-in kit accounts:

- Contact our customer service team at 800-338-8407 to order a kit. Customer service will require basic client and contact information to create your account.

- The kit, materials, and shipping are $70, charged at the time of ordering.

- The processing of each mail-in kit is $460, charged one business day after the kit arrives to the lab.

- A storage option for your sample is required and storage terms range from monthly ($40) to ten years ($2700). Storage is selected on the Billing Agreement received with the kit. Terms can be extended or terminated early (fee may apply) following submission of the Billing Agreement.

- Upon receipt of the kit, refrigerate the kit until you plan on using it. When you are ready to use the kit, let it warm up to room temperature.

- The following documents must be returned with the kit:

  • Off Site Specimen Collection Record
  • Storage Client Information
  • Client Questionnaire
  • Semen Evaluation Freeze and Storage Agreement
  • Billing Agreement (with a storage option selected)
  • Medical and Genetic Tests for Client Depositor

- Follow the "Instructions for Collection and Overnight Return Shipment of Semen Specimens" to ensure you sample is collected and returned properly.

- The lab will process your kit and email the results to you one business day after receiving the kit.

- Bloodwork outlined on the "Medical and Genetic Tests for Client Depositor" form is required by the FDA. Failure to submit proper bloodwork can result in quarantine fees outlined on the Billing Agreement.

- Once your sample is received at the lab, it will be stored if any living cells are observed in the sample.

- In order to use your stored sample, you must contact customer service to place an order for shipment. A Use and Release form is required for shipping that outlines who will use the vials and where they will ship.

- If you have not used your sample at the end of your storage term, you may extend/renew your storage term. No samples will be destroyed by the lab unless a "Semen Specimen Destruction Authorization" has been notarized and submitted. This document is irrevocable once it has been submitted.

Do you work with clients before GAHT?

Gender Affirming Hormone Therapy (GAHT) Client Fertility Preservation

Fairfax Cryobank is honored to support fertility preservation efforts for people preparing to go through GAHT. 

In order to bank a sample at the Cryobank, one must be able to produce a sample by self-masturbation. If an individual is unable to produce a specimen, they can consult with a specialist about having sperm removed directly from the testicles. Testicular sperm cannot be used in insemination but can successfully fertilize an egg using in vitro fertilization (IVF) or Intracytoplasmic sperm injection (ICSI). Sperm cryopreserved by the Cryobank can be used in intrauterine inseminations (IUI), intracervical insemination (ICI), IVF, or ICSI.

There are two pathways for trans individuals to consider:

  • You are banking for use in the future with a non-sexually intimate recipient, e.g. Gestational Carrier/Surrogate will carry the pregnancy

In this instance you will proceed as a Directed Donor*

  • You are banking for use in the future with a sexually intimate recipient, .g. your future partner will carry the pregnancy

In this instance, you will proceed as a Client Depositor.

Will my insurance cover the cost of sperm banking and will you bill my insurance company directly for reimbursement?

Fairfax Cryobank does not have the facilities to bill insurance companies directly for reimbursement. We can provide you with copies of paid invoices to submit to insurance companies upon request.

Is the freezing process harmful to the sperm cells?

Approximately 50% or more of the sperm cells survive the freezing/thawing process. The fertilization capability of the surviving sperm cells is not jeopardized during the freezing/thawing process.
