For Patients
Review and sign our Terms of Use electronically or by mail. Submit them at least one business day before shipment. If you have questions, our Client Services team is available to help at 800-338-8407.

For Physicians
All orders must be placed through a licensed medical professional. Confirm if your physician already has an account. If not, we will send them the necessary forms. We can also provide referrals to reproductive specialists if needed.
