Fairfax Cryobank Sperm Freezing Kit

Research Study Information

Participation in the sperm banking by mail research study is voluntary and all specimens will be discarded at the conclusion of the study. All donors will produce a semen specimen using the Fairfax Sperm Freezing Kit instructions and ship it to Fairfax Cryobank for evaluation and processing. An Agreement to Participate in a Research Study Document and Release of Information form will be sent to you with the kit and will need to be completed and sent back to Fairfax Cryobank in the return shipment.
